Primary asphyxiating thoracic dystrophy, also known as Jeune syndrome, is a rare autosomal recessive genetic disorder linked to pathogenic gene variants. Although its detailed pathogenesis remains incompletely understood, the resulting thoracic abnormalities can cause serious respiratory problems early in life.

 

The Genetic and Developmental Basis of Jeune Syndrome

Primary Asphyxiating thoracic dystrophy (ATD) is generally recognized as an autosomal recessive skeletal ciliopathy linked to pathogenic variants in genes involved in ciliary function. These variants can disrupt skeletal development, although the detailed mechanism by which they produce different thoracic patterns remains incompletely understood. The specific causative variant is not identified in every patient. Secondary ATD is a separate condition that may develop after previous open chest wall deformity surgery.

These disturbances can affect the normal growth of the ribs and costal cartilages, resulting in abnormal thoracic morphology characterized by a narrowed chest, which may subsequently impact lung development and respiratory function.

 

The Impact and Clinical Manifestations

Children with Jeune syndrome have a narrow, abnormally shaped chest, which can make it difficult for their lungs to expand fully. The narrowed thoracic cage restricts lung expansion and may cause breathing difficulty and hypoxemia. Chronic pulmonary compression and inadequate lung expansion may also predispose patients to recurrent respiratory infections. Many infants show signs of oxygen deficiency at birth, and without treatment, the condition can be life-threatening.

Long-term effects include slow growth, short stature, and unevenly developed arms and legs. In addition to respiratory abnormalities, patients may also present with multisystem organ involvement, leading to complications such as renal failure or hepatic dysfunction. Because of these serious complications, ATD requires careful monitoring and medical intervention to improve outcomes.
